Documents Required for FSSAI Registration
Basic Registration needs photo ID and address proof, a passport photo, and a list of food products. A State or Central License adds Form B, an NOC from your local authority, a premises layout plan, and — for import/export businesses — an Import Export Code.

For Basic Registration
Photo ID and address proof of the applicant (Aadhaar, PAN, Voter ID, or passport), a recent passport-size photograph, a declaration form, and a list of the specific food products or categories your business deals in.

Form B, an NOC from your local municipal or panchayat authority, a layout plan of your premises, a list of machinery or equipment if you manufacture or process food, and incorporation or partnership documents if you're not a sole proprietorship. Do I need a water test report for FSSAI registration?
Only if you're a manufacturing or processing unit that uses water as an ingredient or in food processing — a water testing report from a recognised lab is required for State and Central License applications in that case. Basic Registration and non-manufacturing businesses generally aren't asked for it, though your local authority can still request it.
